Sharing Questionnaires
Several ways to get a form to a client.
Send by email
Click Send from the questionnaire page:
- Pick a client or enter an email
- Customize the subject and message
- Add CC / BCC recipients
- BCC yourself
Share by link
Create a request without sending an email. You get a link to share however you like.
Request settings
When sharing:
- Link to a client, booking, or lead for organization
- Allow edit after submit - Let clients update answers after submitting
- Expiry - Set a deadline for completing the form
What the client sees
A clean form page with your business name and logo, the title and description, an estimated completion time, all the fields, and a submit button.
After submitting, they see a confirmation. If "allow edit after submit" is on, they can come back and update answers.
Statuses
| Status | What it means |
|---|---|
| Draft | Created but not sent |
| Awaiting response | Sent, no answer yet |
| Completed | Client submitted |
| Cancelled | You cancelled it |
| Expired | Deadline passed |
Intake questionnaires
If you attach a questionnaire to a booking service, it shows up inline in the booking form. Clients fill it out as part of booking. Responses are linked to the resulting lead or booking.
