Building a Form

Go to Questionnaires and click New Questionnaire.

Basic setup​

  • Title - What clients see at the top of the form
  • Description - Optional instructions or context shown below the title

Adding fields​

Click Add Field to add questions. Each field has:

  • Type - What kind of input (see Field Types)
  • Label - The question text
  • Description - Helper text below the field
  • Placeholder - Hint text inside the input
  • Required - Whether the client must answer
  • Options - For select fields, the choices available
  • Conditional logic - Show this field only when a condition is met (see Conditional Logic)

You can have up to 100 fields per questionnaire.

Reordering​

Drag and drop fields to change the order. This is the order clients see.

Editing and archiving​

Edit a questionnaire anytime. Changes only affect future sends; existing requests keep the version they were sent with.

To retire a questionnaire, archive it. It's hidden from your list but all existing requests and responses stay accessible.

Completion time​

CoreHue estimates how long the form takes to fill out and shows this to clients. It's based on the number of fields.
