Viewing Responses

Questionnaire requests page

Go to Questionnaire Requests to see all your sent forms and their statuses. This is separate from the Questionnaires page (which shows your form library/definitions).

Filter the requests list by:

  • Status (awaiting response, completed, cancelled, expired)
  • Source (manual, booking, lead)
  • Client
  • Booking

Viewing a submission

Click any request to see:

  • The questionnaire as it was when sent (not the current version, in case you have edited it since)
  • Status and dates (sent, viewed, submitted)
  • Client info with links to their profile and booking
  • Every response with the question label
  • Any uploaded files (viewable and downloadable)

Intake responses

For questionnaires filled out during booking, responses are viewable from the lead or booking detail page. You do not need to navigate to the requests page separately; the responses are right there in context.

File uploads

Files uploaded by clients are stored securely. You can view or download each one from the submission page.
