Questionnaire templates

Questionnaire templates are your reusable forms, the questions you build once and send to as many clients as you like. This is your form library.

You'll find it under Templates → Questionnaires, where each form is listed with its Title and when it was last Updated.

Starter questionnaires​

New accounts come with a starter set so you have forms ready on day one. You'll always get Project Inquiry and Session Feedback, plus more based on what you photograph, like Wedding Day Details, Portrait Session Details, Engagement, Family, Newborn, Event Details, Commercial Photography Brief, or Boudoir.

Create a template​

Click New template to open the builder and start a new form. For the full walkthrough, adding items, choosing field types, and setting display logic, see Building a form.

View a template​

Open any form to see its detail. It lays out every question with:

  • A type pill showing the field type.
  • A required pill on must-answer questions.
  • The options for choice questions.
  • A summary of any conditional logic.

From here you can Send questionnaire, Edit, or Delete.

Edit a template​

Click Edit to change a form anytime. Edits only affect forms you send from now on, questionnaires already out with clients keep the version they were sent with.

Send a template​

Choose Send questionnaire to share it. You can send by email or copy a link, to a client, an inquiry, or a booking. See Sharing for the details.

Delete a template​

Deleting a form removes it from your library, but it keeps your past requests: the questionnaires you've already sent stay in your history, answers and all. Read them anytime in Viewing responses.
